CVC Procedure

Step 1:Employee Information
Complete the CVC Request Form Employee Information. 

Be sure to complete all information in the first section of the form as we are unable to process incomplete forms.

Step 2:Supervisor Approval
Please have your supervisor approve and sign the form in the appropriate area of the form.  Without your supervisor's approval we will be unable to process your application.

Step 3:Submit Form
Once both you and your supervisor have completed the first two sections of the form, return the CVC Request form to the Employee Benefits Unit.  You have the following options:

    Mail or Interoffice to:   Employee Benefits Service Unit
                                      651 Pine Street, 5th Floor
                                      Martinez, CA  94553 or 


Step 4:EBSU Processing
Once our office has received the form we will review and submit the information to VSP.

Step 5:Receipt of Authorization from VSP
VSP will send you an authorization letter (if you include the doctor's information, it will be faxed directly to that office and you will not receive a letter).  DO NOT MAKE AN APPOINTMENT UNTIL EITHER YOU or YOUR DOCTOR RECEIVE THE AUTHORIZATION. 

Step 6:Make your appointment
Make your appointment within 60 days of the authorization.
