VSP Approval:

• If doctor's information is included on the form, the doctor will receive your authorization directly by fax in 5 to 7 business days, no authorization will be sent to the employee.
• If doctor's information is blank or incomplete, VSP will send the letter of Authorization to the employee in 7 to 10 business

Does my authorization expire?

Yes. You have 60 days from the date of authorization to use services.
