C.18    AUTHORIZE the Auditor-Controller to reduce the Designation for Litigation and Audit Exceptions; and AUTHORIZE the transfer of appropriations in the amount of $280,863 to the Office of the Sheriff for a retroactive, wage payment to members of the Deputy Sheriff’s Association. APPROVE and AUTHORIZE execution of agreements between the County and the following agencies for receipt of funds and/or services:

 

C.20    APPROVE and AUTHORIZE the County Administrator, or designee, to apply for and accept a grant in the amount of $250,000 on behalf of the County’s Zero Tolerance for Domestic Violence Initiative from the U.S. Department of Health and Human Services, Administration for Children and Families, Office of Refugee Resettlement for the Rescue & Restore Victims of Human Trafficking Regional Program. (Catalog of Federal Domestic Assistance No. 93.598) (No County match) (All Districts)

 

C.21    APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to apply for and accept funding from the Community Services Block Grant Program in an amount not to exceed $25,000 to support the Contra Costa County Service Integration Program’s Nystrom Involving Parents Program for the period April 1, 2009 through March 31, 2010.  (No County match) (All Districts)

 

C.22    APPROVE and AUTHORIZE the Employment and Human Services Director, or designee, to execute a contract amendment with the California Department of Aging, to increase the payment limit by $77,251 to a new payment limit of $3,829,014 for Title III and Title VII programs of the Older Americans Act for the period July 1, 2008 through June 30, 2009.  (No additional County match) (All Districts)

 

C.23    APPROVE and AUTHORIZE the Conservation and Development Director, or designee, to execute the second amendment to the Fee Agreement with Pacific Gas & Electric to modify construction schedule and amend fee structure in connection with the compliance and enforcement-related services provided at the Gateway Generating Station Project. (100% fees from PG&E) (District V)

 

C.24    APPROVE and AUTHORIZE the General Services Director, or designee, to execute a contract amendment with Loving & Campos Architects, Inc., effective January 27, 2009 to increase the payment limit by $500,000 to a new payment limit of $1,000,000 and to specify a termination date of December 31, 2010 for additional as-needed architectural services for Contra Costa County Fire Protection District Projects. (Budgeted) (All Districts)

 

C.25    APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to execute a contract with the Housing Authority to pay the County $345,100 to provide contract law enforcement services for the period April 1, 2008 through March 31, 2009. (100% Revenue) (District II)

 

APPROVE and AUTHORIZE execution of agreements between the County and the following parties as noted for the purchase of equipment and/or services:

 

C.26    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract with the Board of Trustees of the University of Illinois, on behalf of its University of Illinois at Chicago – School of Public Health for its Center for Advancement of Distance Education in an amount not to exceed $65,000 to provide emergency preparedness planning, exercise, and training with regard to County’s Strategic National Stockpile Plan for the period May 1, 2008 through March 31, 2009, including mutual indemnification  for any claims arising out of the performance of this contract. (100% Federal Department of Homeland Security) (All Districts) 

 

C.27    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Adolescent Treatment Centers, Inc., in an amount not to exceed $210,000 to provide day rehabilitation and mental health services for seriously emotionally disturbed dually-diagnosed adolescents at its Thunder Road Facility for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $105,000. (50% Federal Medi-Cal, 50% State Early and Periodic Screening, Diagnosis, and Treatment) (All Districts) 

 

C.28    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with La Cheim School, Inc., in an amount not to exceed $280,000 to provide a residential day treatment program and mental health services for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $140,000. (50% Federal Medi-Cal, 45% State Early and Periodic Screening, Diagnosis, and Treatment, 5% Mental Health Realignment) (All Districts) 

 

C.29    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Bay Area Community Resources, Inc., in an amount not to exceed $550,000 to provide school-based mental health services for severely emotionally disturbed students and their families for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2008 in an amount not to exceed $275,000. (50% Federal FFP Medi Cal, 45% State Early and Periodic Screening, Diagnosis, and Treatment, 5% Mental Health Realignment) (All Districts) 

 

C.30    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with La Cheim School, Inc., effective January 1, 2009, to increase the payment limit by $300,000 to a new payment limit of $714,638 to provide additional full-day intensive day treatment services with no change in the original term of July 1, 2008 through June 30, 2009, and to increase the automatic extension payment limit by $150,000 to a new total of $357,319. (25% Federal Medi-Cal, 23% State Early and Periodic Screening, Diagnosis, and Treatment, 50% Individuals with Disabilities Education Act, 2% Mental Health Realignment) (All Districts) 

 

C.31    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Young Men’s Christian Association of the East Bay (dba YMCA of the East Bay), in an amount not to exceed $402,168 to provide school-based mental health services to seriously emotionally disturbed students in West County for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $201,084. (50% Federal Medi-Cal, 46% State Early and Periodic Screening, Diagnosis, and Treatment, 4% West Contra Costa Unified School District) (Districts I and II) 

 

C.32    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with FamiliesFirst, Inc., in an amount not to exceed $215,509 to provide school-based mental health services to seriously emotionally disturbed students in the Mount Diablo Unified School District, for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $107,755. (23% Federal Medi-Cal, 18% State Early and Periodic Screening, Diagnosis, and Treatment, 28% Mount Diablo Unified School District, 30% Individuals with Disabilities Education Act) (District IV) 

 

C.33    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Desarrollo Familiar, Inc., in an amount not to exceed $965,766 to provide implementation of County’s Mental Health Active Community Supports and Service Teams Project for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $482,883. (1% Federal Medi-Cal, 1% State Early and Periodic Screening, Diagnosis, and Treatment, 98% Mental Health Services Act) (All Districts) 

 

C.34    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with the West Contra Costa Youth Services Bureau, a non-profit corporation, in an amount not to exceed $750,000 to provide wraparound mental health services for youth for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $375,000. (48% Federal FFP Medi-Cal, 43% by State Early and Periodic, Diagnosis, Screening and Treatment, 4% Individuals with Disabilities Education Act, 5% Mental Health Realignment) (All Districts) 

 

C.35    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Young Men’s Christian Association of the East Bay, Inc., (dba YMCA of the East Bay) in an amount not to exceed $403,000 to provide on-school-site counseling services for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $201,500. (50% Federal Medi-Cal, 40% State Early and Periodic Screening, Diagnosis, and Treatment, 10% Mental Health Realignment) (All Districts) 

 

C.36    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract extension with Westcoast Children’s Clinic to extend the term of the automatic extension from December 31, 2008 through March 31, 2009, with no change in the payment limit of $298,367 or the automatic extension payment limit of $198,911. (40% Federal Medi-Cal, 35% State Early and Periodic Screening, Diagnosis, and Treatment, 20% Individuals with Disabilities Education Act, 5% Mental Health Realignment) (All Districts) 

 

C.37    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Asian Pacific Psychological Services in an amount not to exceed $187,324 to provide school-based mental health services to seriously emotionally disturbed students in West County for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $93,662. (50% Federal Medi-Cal, 45% State Early and Periodic Screening, Diagnosis, and Treatment, 5% West Contra Costa Unified School District) (Districts I and II) 

 

C.38    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with the West Contra Costa Youth Service Bureau in an amount not to exceed $428,780 to provide school-based mental health services to seriously emotionally disturbed students in West County for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $214,390. (50% Federal Medi-Cal, 46% State Early and Periodic Screening, Diagnosis, and Treatment, 4% West Contra Costa Unified School District) (Districts I and II)  

 

 

C.39    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Bay Area Community Resources, Inc., in an amount not to exceed $262,920 to provide school-based mental health services to seriously emotionally disturbed students in West County for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009, in an amount not to exceed $131,460. (50% Federal Medi-Cal, 45% State Early and Periodic Screening, Diagnosis, and Treatment, 5% West Contra Costa Unified School District) (Districts I and II) 

 

C.40    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a contract amendment with Youth Homes Inc., effective February 1, 2009, to increase the payment limit by $310,000 to a new payment limit of $1,710,000 to provide residential treatment services to additional severely emotionally disturbed children, with no change in the original term of July 1, 2008 through June 30, 2009, and to increase the automatic extension payment limit by $155,000 to $855,000. (45% Federal Medi-Cal, 40% State Early and Periodic Screening, Diagnosis, and Treatment, 10% Individuals with Disabilities Act, 5% Mental Health Realignment) (All Districts) 

 

C.41    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with Mental Health Consumer Concerns of Alameda County, Inc., in an amount not to exceed $323,526 to provide support services and training programs for County’s mental health clients for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $166,763 (100% Mental Health Realignment) (All Districts) 

 

C.42    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with the West Contra Costa Youth Services Bureau in an amount not to exceed $790,000 to provide wraparound services to severely emotionally disturbed children for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $395,000. (50% Federal Medi-Cal, 48% State Early and Periodic, Screening, Diagnostic and Treatment, 2% Mental Health Realignment) (All Districts) 

 

C.43    APPROVE and AUTHORIZE the Health Services Director, or designee, to execute a novation contract with We Care Services for Children in an amount not to exceed $880,000 to provide day treatment services for high risk, delayed or emotionally disturbed children for the period July 1, 2008 through June 30, 2009, with a six-month automatic extension through December 31, 2009 in an amount not to exceed $440,000. (50% Federal Medi-Cal, 48% State Early and Periodic Screening, Diagnosis and Treatment, 2% Mental Health Realignment) (All Districts)
